You are entitled to one
additional
volume from the set.
Please provide me

of the additional volume you wish to receive.
There is a space on the Contributor's Agreement for this information.
3. A complete set
of agreements
for each volume will be sent to the volume editor(s) for his/hers/their
files. Each contributor's contact information and optional
complementary
volume title selected will be forwarded to the publisher to facilitate
the delivery of complimentary off-prints and volumes.
4. When the
volumes are about
to go to press I will post separate order forms for off-prints and
books.
Volume editors and contributors may print-off and use these forms to
order
any additional books or off-prints above their complementary allotment.
