  2.  Complete the Contributor's Agreement and mail the original

You will automatically receive a copy of the volume in which your chapter appears. 
You are entitled to one additional volume from the set.
Please provide me with the name of the additional volume you wish to receive.
There is a space on the Contributor's Agreement for this information.

3.  A complete set of agreements for each volume will be sent to the volume editor(s) for his/hers/their files.  Each contributor's contact information and optional complementary volume title selected will be forwarded to the publisher to facilitate the delivery of complimentary off-prints and volumes.

4.  When the volumes are about to go to press I will post separate order forms for off-prints and books.  Volume editors and contributors may print-off and use these forms to order any additional books or off-prints above their complementary allotment.
